Create PDF Form adds fillable fields (text, checkbox, radio, combo, listbox, signature) to a PDF, placed either by an exact `rect` or by anchoring to the first match of `anchor.text` on the page. An anchor that doesn't match any text on the page returns a 422 rather than silently placing the field at a default position.
